ABSTRACT

Background/Aims@#A re-increasing trend of thyroid cancer since 2015 has been observed despite a similar examination rate, and the incidence of thyroid cancer among young adults continues to rise. @*Methods@#This study used data from the Korean National Health Insurance Service. Individuals 20–39 years of age who underwent ≥ 4 health checkups from 2009–2013 were enrolled and followed throughout 2019. To quantify the metabolic burden, groups were divided by the number of diagnoses of metabolic syndrome across four consecutive health examinations. @*Results@#Among the study population (n = 1,204,646), 5,929 (0.5%) were diagnosed with thyroid cancer during a follow- up period of 5 years. The hazard ratio (95% confidence interval) values of thyroid cancer occurrence according to the number (1–4) of diagnoses of metabolic syndrome across the four health examinations compared to the group without metabolic syndrome were significantly greater, as follows: 1.12 (1.02–1.23), 1.25 (1.10–1.42), 1.33 (1.15–1.55), and 1.48 (1.25–1.75) (p for trend < 0.01), respectively. Each component of metabolic syndrome showed a significant increase in hazard ratio according to the number of diagnoses except for impaired fasting glucose criteria. @*Conclusions@#Cumulative exposure to metabolic syndrome was associated with thyroid cancer risk in young adults.

ABSTRACT

Purpose@#The purpose of this study is to identify the educational needs of virtual reality simulations that can be applied to novice nurses during the waiting period before starting work in a hospital. @*Methods@#A convergent mixed methods was used. The survey data were collected from 230 novice nurses, and a focus group interview was conducted with 6 new nurses. The data were collected from November 2022 to January 2023. Descriptive statistics, a frequency analysis, independent t-test, and an Importance-Performance Analysis were performed using SPSS 24.0. @*Results@#Appropriate topics for virtual reality simulation education were indicated to be medications and intravenous injections, which are high priority topics in quantitative and qualitative research. The novice nurses wanted group activity training three to four times a week for two weeks before beginning work in a hospital. They also wanted an immersive virtual reality system based on a real hospital environment. @*Conclusion@#Based on the above results, this study provides basic data for the development of a virtual reality simulation education that can improve the adaptation of novice nurses to clinical practice. A strategy was suggested to utilize the waiting time before beginning work in a hospital as educational time.

ABSTRACT

A 58-year-old woman visited the hospital complaining of fatigue and indigestion lasting for more than 3 months. She had no medical history other than taking a calcium plus vitamin D supplement for osteopenia. The initial blood test showed a high calcium level of 14.0 mg/dL. Additional tests were performed to differentially diagnose hypercalcemia. The blood test results were as follows: serum parathyroid hormone (PTH)=247.0 pg/mL, PTH-related peptide <1.0 pg/mL, phosphorous=2.6 mg/dL, 25-hydroxy-vitamin D=14.5 pg/mL, creatinine=1.09 mg/dL, and 24 hr urine calcium=215 mg/dL. A 4.5 cm sized cystic lesion on the intra-thyroidal space was confirmed on neck sonography and 4-dimensional parathyroid computed tomography, but technetium-99m methoxyisobutylisonitrile parathyroid scintigraphy showed equivocal results. After removal of the cystic lesion, serum calcium and PTH were normalized, and parathyroid lipoadenoma was confirmed in the postoperative pathology. Clinical features of parathyroid lipoadenoma are known to be similar to common parathyroid adenoma, but imaging studies often report negative findings. Therefore, it is necessary to better understand this rare disease for the differential diagnosis. For the final diagnosis and treatment of this disease, parathyroidectomy with intraoperative PTH measurement may be required.

ABSTRACT

Background@#Persistence with denosumab in male patients has not been adequately investigated, although poor denosumab persistence is associated with a significant risk of rebound vertebral fractures. @*Methods@#We retrospectively evaluated 294 Korean male osteoporosis patients treated with denosumab at three medical centers and examined their persistence with four doses of denosumab injection over 24 months of treatment. Persistence was defined as the extent to which a patient adhered to denosumab treatment in terms of the prescribed interval and dose, with a permissible gap of 8 weeks. For patients who missed their scheduled treatment appointment(s) during the follow-up period (i.e., no-shows), Cox proportional regression analysis was conducted to explore the factors associated with poor adherence. Several factors were considered, such as age, prior anti-osteoporotic drug use, the treatment provider’s medical specialty, the proximity to the medical center, and financial burdens of treatment. @*Results@#Out of 294 male patients, 77 (26.2%) completed all four sequential rounds of the denosumab treatment. Out of 217 patients who did not complete the denosumab treatment, 138 (63.6%) missed the scheduled treatment(s). Missing treatment was significantly associated with age (odds ratio [OR], 1.03), prior bisphosphonate use (OR, 0.76), and prescription by non-endocrinologists (OR, 2.24). Denosumab was stopped in 44 (20.3%) patients due to medical errors, in 24 (11.1%) patients due to a T-score improvement over –2.5, and in five (2.3%) patients due to expected dental procedures. @*Conclusion@#Our study showed that only one-fourth of Korean male osteoporosis patients were fully adherent to 24 months of denosumab treatment.

ABSTRACT

Molar-incisor malformation (MIM) is a newly reported dental anomaly with molar root deformity and incisor crown defects. MIM-affected teeth may cause severe pain with no apparent tooth caries. Since the affected molars clinically appear normal, radiographs are recommended for accurate diagnosis on the first visit. Since MIM-affected patients are in mixed dentition, timely and appropriate interventions are needed to avoid unnecessary pain and complicated clinical issues. This report was written to describe two patients who had MIM in early mixed dentition and report their 2-year follow-ups.

ABSTRACT

Background@#High-density lipoprotein cholesterol (HDL-C) plays an important role in the reverse cholesterol transport pathway and prevents atherosclerosis-mediated disease. It has also been suggested that HDL-C may be a protective factor against cancer. However, an inverse correlation between HDL-C and cancer has not been established, and few studies have explored thyroid cancer. @*Methods@#The study participants received health checkups provided by the Korean National Health Insurance Service from 2009 to 2013 and were followed until 2019. Considering the variability of serum HDL-C level, low HDL-C level was analyzed by grouping based on four consecutive health checkups. The data analysis was performed using univariate and multivariate Cox proportional hazard regression models. @*Results@#A total of 3,134,278 total study participants, thyroid cancer occurred in 16,129. In the crude model, the hazard ratios for the association between repeatedly measured low HDL-C levels and thyroid cancer were 1.243, 1.404, 1.486, and 1.680 (P for trend <0.01), respectively, which were significant even after adjusting for age, sex, lifestyle factors, and metabolic diseases. The subgroup analysis revealed that low HDL-C levels likely had a greater impact on the group of patients with central obesity (P for interaction= 0.062), high blood pressure (P for interaction=0.057), impaired fasting glucose (P for interaction=0.051), and hyperlipidemia (P for interaction=0.126). @*Conclusion@#Repeatedly measured low HDL-C levels can be considered a risk factor for cancer as well as vascular disease. Low HDL-C levels were associated with the risk of thyroid cancer, and this correlation was stronger in a metabolically unhealthy population.

ABSTRACT

A durable normoglycemic state was observed in several studies that treated type 2 diabetes mellitus (T2DM) patients through metabolic surgery, intensive therapeutic intervention, or significant lifestyle modification, and it was confirmed that the functional β-cell mass was also restored to a normal level. Therefore, expert consensus introduced the concept of remission as a common term to express this phenomenon in 2009. Throughout this article, we introduce the recently updated consensus statement on the remission of T2DM in 2021 and share our perspective on the remission of diabetes. There is a need for more research on remission in Korea as well as in Western countries. Remission appears to be prompted by proactive treatment for hyperglycemia and significant weight loss prior to irreversible β-cell changes. T2DM is not a diagnosis for vulnerable individuals to helplessly accept. We attempt to explain how remission of T2DM can be achieved through a personalized approach. It may be necessary to change the concept of T2DM towards that of an urgent condition that requires rapid intervention rather than a chronic, progressive disease. We must grasp this paradigm shift in our understanding of T2DM for the benefit of our patients as endocrine experts.

ABSTRACT

The purpose of this study was to compare surface hardness between titanium-nitride coated crowns (TiNCs) and stainless steel crowns (SSCs), and to evaluate the corrosion resistance and color sustainability of TiNCs. Ten TiNCs and 10 SSCs were used for the hardness test. Measurement was performed 30 times for each type of crowns, and the mean values were compared. Metallic raw material plates (before being processed into crowns) of TiNCs and SSCs were prepared for the corrosion resistance test. The total amounts of metal ion releases in the test solution were detected by inductively coupled plasma-optical emission spectrometry. Five TiNCs were subjected to the color sustainability test by applying repetitive brushing forces. The mean hardness values of TiNC group and SSC group were 395.53 ± 105.90 Hv and 278.70 ± 31.45 Hv respectively. Hardness of TiNCs were significantly higher than that of SSCs. The total amounts of metal ion releases from the materials of TiNCs and SSCs satisfied the criterion in International Organization for Standardization 22674. The results mean that TiNCs and SSCs were not harmful in an acidic environment. The golden coating was stable against the repetitive physical stimulations for a given period time.

ABSTRACT

Background@#Romosozumab has shown significant improvement in bone mineral density (BMD) in previously reported trials. However, BMD reflects only bone strength and does not offer insight into the bone microarchitecture. The trabecular bone score (TBS) is a non-invasive tool used to assess bone microarchitecture. Several previous studies have evaluated the efficacy of anti-osteoporotic agents using the TBS. However, data regarding the potency of romosozumab based on the TBS is lacking. This retrospective observational cohort study demonstrated the impact of romosozumab use on the TBS. @*Methods@#The primary outcome was the percentage change in the TBS from baseline to post-treatment. Postmenopausal osteoporosis patients were followed up for 6 and 12 months after romosozumab (210 mg monthly, N =10) and denosumab (60 mg every 6 months, N=21) or ibandronate (150 mg monthly, N=24) treatments, respectively. Patients who had previously used osteoporosis medications were included, if any the washout period was sufficient. @*Results@#The percentage change in TBS from baseline to post-treatment was 2.53±2.98% (6 months, N=10; P=0.04), 0.59%±3.26% (12 months, N=21; P=0.48), and -0.45±3.66% (12 months, N=24; P=0.51) in the romosozumab, denosumab, and ibandronate groups, respectively. Romosozumab demonstrated a noticeable increase in TBS, although it did not reach the least significant change (5.8%) in TBS. @*Conclusions@#Romosozumab improved the TBS in postmenopausal women with osteoporosis. TBS may be potentially useful for monitoring romosozumab treatment.

ABSTRACT

Background and Objectives@#The lack of known elevated biomarkers in cancer surveillance is a challenge for diagnostics. However, few studies have assessed the proportion of patients with medullary thyroid cancer (MTC) that presented with elevated serum carcinoembryonic antigen (CEA) level. @*Materials and Methods@#This was a retrospective study of 115 patients treated for MTC at Samsung Medical Center between 1995 and 2017. Serum calcitonin and CEA levels that were available at one of the following time points were included: (a) within 3 months before the initial surgery, (b) when a structural recurrence was confined to the neck, or (c) when a distant metastasis was identified during follow-up. Considering the high false-positive rate of serum CEA testing, 5 ng/mL was selected as the cut-off threshold. @*Results@#The sensitivity of CEA at the time of diagnosis was 54%, but it significantly increased with anatomical stage, from 20% in stage I to 88% in stage IV (p for trend <0.001). At the time of diagnosis, larger tumor size, more frequent gross extra-thyroidal extension, more frequent N1b, and a higher number of metastatic lymph nodes were significantly observed in CEA-positive patients compared with CEA-negative patients (p<0.05). The sensitivity of CEA at the time of recurrence was 56% for local recurrence and 80% for distant recurrence. @*Conclusion@#The CEA value was associated with anatomical stage as a biomarker for MTC and could be used to predict poor prognosis. However, serum CEA testing plays a limited role in diagnosis and follow-up of MTC because it can show normal values even in advanced disease.

Background@#Endoscopic surveillance after total gastrectomy (TG) for gastric cancer is routinely performed to detect tumor recurrence and postoperative adverse events.However, the reports on the clinical benefits of endoscopic surveillance are ambiguous. We investigated the clinical benefit of endoscopic surveillance after TG for gastric cancer. @*Methods@#We analyzed 848 patients who underwent TG with R0 resection for gastric cancer between 2011 and 2012 (380 early gastric cancer and 468 advanced gastric cancer) and underwent regular postoperative surveillance with endoscopy and abdominopelvic computed tomography (CT) with contrast. @*Results@#Median follow-up periods were 58 months for both endoscopy (range, 3–96) and abdominopelvic CT (range, 1–96). Tumor recurrence occurred in 167 patients (19.7%), of whom seven (4.2%) were locoregional recurrences in the peri-anastomotic area (n = 5) or regional gastric lymph nodes (n = 2). Whereas the peri-anastomotic recurrences were detected by both endoscopy and abdominopelvic CT, regional lymph node recurrences were only detected by abdominopelvic CT. Out of the 23 events of postoperative adverse events, the majority (87%) were detected by radiologic examinations; three events of benign strictures in the anastomotic site were detected only by endoscopy. @*Conclusion@#Endoscopic surveillance did not have a significant role in detecting locoregional tumor recurrence and postoperative adverse events after TG with R0 resection for gastric cancer. Routine endoscopic surveillance after TG may be considered optional and performed according to the capacities of each clinical setting.

Moreover, the social norm of smoking has changed. These changes have prompted the tobacco industry to evolve their products and business model. There have been several evolutions in tobacco products. Firstly, new tobacco products, such as electronic cigarettes and heated tobacco products were introduced into markets that were previously dominated by conventional cigarettes. Thus, smokers are able to select alternative tobacco products. Secondly, new devices for tobacco use, including universal serial bus, smart watch, and car key devices, have been developed and released. These devices are popular among females and youth, who wish to hide their smoking behavior. Thirdly, nicotine, which is used in tobacco products, has also evolved. Typically, ‘free-base nicotine’ has been the main form used in tobacco products, because it helps to deliver nicotine quickly to the smoker's brain. However, a new form of nicotine, ‘nicotine salt’, is used in electronic cigarettes. It can deliver a more highly concentrated dose of nicotine to the smoker’s brain. To regulate these evolving products, laws are required to prevent the easy access of new tobacco products in the Korean market and therefore, a new organization for monitoring the evolution of tobacco products is required. Additionally, we need to encourage young people and scholars to develop interest in tobacco product control, thus ensuring adequate regulation of new tobacco products.

ABSTRACT

Although the Korean government introduced a tobacco tax increase in 2015 and pictorial health warnings on tobacco packages in 2016, the smoking prevalence among Korean adult men has remained steady. More efforts and effective tobacco control policies are needed to tackle the highest smoking prevalence among the member countries of the Organization for Economic Cooperation and Development. Plain or standardized packaging of tobacco products has been evaluated as an effective tobacco control policy after its introduction in Australia in 2012. The purpose of this study was to raise awareness of the need to introduce plain packaging of tobacco products in Korea. This study presents information on the following topics: what plain packaging of tobacco products is; the purposes of this policy; the reasons why we need to implement this policy in Korea; the scientific evidence regarding the effects of plain packaging of tobacco products; international developments in the plain packaging of tobacco products; and what we need to consider in order to prepare for the implementation of this policy in Korea. We propose that introducing plain packaging of tobacco products would be beneficial for reinforcing tobacco control policies in Korea and would simultaneously contribute to implementing articles 11 and 13 of the Framework Convention on Tobacco Control. Further studies must be carried out to develop a plain or standardized packaging design of tobacco products in Korea and to prevent the tobacco industry from interrupting the introduction of this policy in Korea, based on other countries' experience.

ABSTRACT

This study aimed to investigate the correlation between the inclinations of the incisors and lips in mixed dentition aged 7 – 11 years. Lateral cephalometric radiographs of 353 children were analyzed to measure the upper incisor inclination, lower incisor inclination, lower-nasolabial angle, and mentolabial angle in relation to the skeletal malocclusion classification. The measurements were then assessed using the Kruskal-Wallis test, Pearson correlation, and multiple regression analysis.There were significant negative correlations between the inclination of the upper incisors and lower-nasolabial angle in all classes of skeletal malocclusion. There was a negative correlation between the inclination of the lower incisors and mentolabial angle; however, class Ⅱ malocclusion had a significant positive correlation.This study identified the factors that affect lip inclination and verified their associations.

ABSTRACT

Tuberculosis rarely involves the esophagus, and most esophageal tuberculosis occurs secondary to adjacent tuberculous lymphadenitis. An esophago-mediastinal fistula is a very unusual complication of tuberculous lymphadenitis. Herein, we report a case of an esophago-mediastinal fistula due to tuberculous lymphadenitis. A 28-year-old woman who had dysphagia was assessed by chest computed tomography, endoscopy, esophagogram, and a lymph node biopsy. An esophago-mediastinal fistula was found and an antituberculous agent was considered initially. However, because of her severe dysphagia, she managed with endoscopic clipping as an alternative. However, the fistula remained on follow-up esophagography. A gastrostomy was eventually performed, and she was treated with an antituberculous agent. The fistula had closed after 4 weeks of medication. Antituberculous agents are the mainstay treatment for esophago-mediastinal fistula due to tuberculous lymphadenitis; endoscopic clipping may be a complementary treatment.

ABSTRACT

PURPOSE: The purpose of this study was to identify the influences of health beliefs on fall prevention behavior among adult patients who had abdominal surgery. METHODS: Data were collected from 136 patients who had undergone an abdominal surgery within the past 5 days. The data collection period was from September 21 to October 25, 2017. The health belief measurement tool, modified and reviewed by experts, and the fall prevention guideline developed by the Korean Nurses Association and revised by Park were used. Data were analyzed using descriptive statistics, t-test, ANOVA, Pearson correlation and stepwise multiple regression analysis. RESULTS: The Health Belief Model explained 45.7% of the variance in fall prevention behavior among adult patients who had abdominal surgery. Perceived susceptibility and perceived benefits had significant influence on fall-prevention behavior. CONCLUSION: Tailored educational programs which put emphasis on the perceived susceptibility and perceived benefits of fall prevention need to be developed.

ABSTRACT

Although the Korean government introduced a tobacco tax increase in 2015 and pictorial health warnings on tobacco packages in 2016, the smoking prevalence among Korean adult men has remained steady. More efforts and effective tobacco control policies are needed to tackle the highest smoking prevalence among the member countries of the Organization for Economic Cooperation and Development. Plain or standardized packaging of tobacco products has been evaluated as an effective tobacco control policy after its introduction in Australia in 2012. The purpose of this study was to raise awareness of the need to introduce plain packaging of tobacco products in Korea. This study presents information on the following topics: what plain packaging of tobacco products is; the purposes of this policy; the reasons why we need to implement this policy in Korea; the scientific evidence regarding the effects of plain packaging of tobacco products; international developments in the plain packaging of tobacco products; and what we need to consider in order to prepare for the implementation of this policy in Korea. We propose that introducing plain packaging of tobacco products would be beneficial for reinforcing tobacco control policies in Korea and would simultaneously contribute to implementing articles 11 and 13 of the Framework Convention on Tobacco Control. Further studies must be carried out to develop a plain or standardized packaging design of tobacco products in Korea and to prevent the tobacco industry from interrupting the introduction of this policy in Korea, based on other countries' experience.
